Send
your completed form to the address provided on the form,
along with a personal or company cheque, or money order.
Please make cheques payable to: Alzheimer Society of
Canada.

your completed form to (416)488-3778. (The fax number
is also on the form.) Please be sure to include your
credit card number, expiry date, your name as it appears
on the credit card and your signature.

can donate by telephone
We
welcome your telephone call at any time and can accept
donations from 9 a.m. - 5 p.m. EST at 1-800-616-8816.
Please have your credit card number available. We accept
VISA, MasterCard and AMEX.
Receipt
and acknowledgement
The
Alzheimer Society would like to thank you for any donation
you can make today to support our work. Your donation
will help to fund research into the cause and cure of
Alzheimer's disease and provide vital services to people
with Alzheimer's disease and their families. Official income
tax receipts are automatically issued for all donations
of $10 or more. If you would like a receipt for less
